HealthPartners My Rewards
HealthPartners My Rewards is a one stop shop for rewards and incentives for participating in programs to help you live healthier.
Members over the age of 18 can earn up to $245 in rewards which are available for a wide range of straightforward member actions that let members quickly make progress on reaching their health and wellness goals, maximizing their care, or learning more about the HealthPartners plan resources.
All rewards come on a pre-loaded gift card, making them easy for members to redeem.
Once you have signed in to your HealthPartners account, at healthpartners.com select the trophy icon near the top of the page to access My Rewards. There, you can find which rewards you’re eligible for as well as a record of those you’ve already earned.

Assist America
Whether you’re traveling abroad or just out of town for the weekend, you can feel confident you’re in good hands when the unexpected happens. Get 24/7 help with Assist America! Omada is a personalized program designed to help you reach your health goals – whether that’s losing weight, lowering your blood pressure, or staying on top of your diabetes. Health Coaching
You have access to free telephonic and online support for tobacco cessation and intensive weight management. Coaches offer personalized decision support to help participants set and achieve their individual goals through a personal action plan. Members may self-enroll at any time, and we also proactively identify members through predictive modeling based on claims or health assessment data.
